Patient Financial Barriers to Medical Cannabis Access
This study aimed to explore patient perspectives on medication access, financial toxicity, and the role of medical cannabis (MC) in advanced cancer. We conducted semi-structured interviews with 20 patients with advanced cancer who were using or considering using MC.
Patients described significant financial risks of accessing MC posttrial as financial toxicity, and explicating the threat it poses to equitable access to MC. They also highlighted the need for policy changes to enable affordable legal access, or access MC through alternative pathways. Our findings underscore the importance of addressing financial barriers to MC access for patients with advanced cancer.

Affordability Barriers to Medical Cannabis Card Purchase
Among 562 patients who were newly certified for medical cannabis between 2017 and 2019, 45% purchased medical cannabis, while 55% did not. Patients who purchased medical cannabis were more likely to be white and have private insurance or Medicare. Unregulated cannabis use and current tobacco use were less common among those who purchased medical cannabis. In multivariable logistic regression analysis, unregulated cannabis use remained negatively associated with purchasing medical cannabis. Patients reported that affordability and dispensary accessibility were their main barriers to purchasing medical cannabis.

To describe the variation among states, we analyzed medical marijuana laws and program characteristics for all 23 states and the District of Columbia that had medical marijuana programs as of July 2015. We found that states with older, less regulated programs had much higher enrollment rates than states with newer, more medicalized programs. The lowest cost for enrolling in a state’s marijuana program was associated with higher enrollment rates. Given variation in state and local regulations, the cost of obtaining a medical marijuana card can vary significantly.
